Why do I need to complete a release of information
Without a completed Release of Information, Daylight Healing cannot disclose any of your protected health information (PHI) to other parties. If you want another person to assist with scheduling your appointment, havie someone else that you would like to pay your bills, or want someone to have access to your clinical records, you will need to complete a Release of Information.
How long is my release of information valid?
Unless otherwise specified, a completed Release of Information is valid for the duration of treatment with Daylight Healing. Patients can select a specific expiration date or event that will terminate the release of information.
How do I revoke my release of information?
A written request is required to revoke a previously authorized release of information. Please complete and return the Revocation of Authorization for Release of Information form if you would like to revoke a a previously authorized ROI. The requested revocation will not be effective until Daylight Healing confirms in writing that the revocation has been received and processed.
Will I have to pay for a copy of my medical records?
You do not have to pay or records that we send directly to you or to another medical professional.
There will be a small fee for printing and mailing medical records. This will be discussed prior to completing the request.
How long will it take to process my request?
We will process your request within 7 business days for direct patient requests.
We will process requests within 10 business days for all others.
To avoid delays, please do not submit duplicate requests.
